DV Packet and Forms

You are filing a Post Judgment Action, which is a formal request to your judge assigned to your case stating that you wish to make a change or request an enforcement of the Final Judgment of Injunction Order that was given to you.

Due to the heavy caseload that Domestic Violence Case Managers have and as well as the Judiciary Staff you may expect a response back from your judge regarding your requested relief in six to eight weeks (8-10 weeks). Please ensure that all documents are fully completed correctly and that all requirements are done to ensure your requested relief be submitted in a timely manner.

CASE MANAGERS ARE NOT ACTING AS YOUR ATTORNEY OR GIVE LEGAL ADVICE. THEY ARE AVAILABLE ONLY TO ENSURE THE PROPER FORMS ARE FILED ACCORDINGLY.

Due to limited court staff and increases in case loads, if a Case Manager is unavailable you have the option to download and complete your documents on your own or click here to use SOFIA (State of Florida Interactive Access). SOFIA is a form builder like TurboTax. If you downloaded and completed the form you must have a public notary notarize your documents in the designated areas of your form. All sections must be TYPED or WRITTEN CLEARLY AND COMPLETED. Use clear sentences to get your voice expressed to the Judge accordingly.

Make sure that you consult with a case manager prior to completing any of the forms to ensure you are selecting the correct packet or form.

A case manager will review your documents for accuracy and then after proper service, submit to the Judge to determine if your requested motion will be granted, denied, or issued a hearing.

You may call (904)255-1075 to check the status of your case after 4 weeks of filing your motion with a Domestic Violence Case Manager.

IF FORMS ARE BEING MAILED BY CERTIFIED MAIL TO THE OTHER PARTY ADDITIONAL TIME MAY BE REQUIRED

You will be opening a PDF Document to print instructions and forms that are attached.
